Birth Certificates in Pennsylvania are issued through the State Department of Health. The cost for each Birth Certifictae is $ 10.00. Please note if you were not born in Pennsylvania, you are not able to obtain a Birth Certificate in Pennsylvania. Information needed to obtain your Birth Certificate is as follows.
• Your FULL name at birth.
• Your Father's FULL name at birth.
• Your Mother's FULL name at HER birth. (maiden name)
• Your Date of Birth
• The Hospital you were born in.
• The County you were born in.
• The Borough, City, or Township you were born in.
• Your current address and phone number.
** If you are a veteran, the spouse of a Veteran, or the minor child (under 18) of a Veteran, you will also need the Veteran's full name, current address, service number, and rank at discharge.
The state provides free Birth Certificates to all veterans, their spouses, and their children under 18 years of age.
The normal processing time for us to obtain your Birth Certificate is four to five business days but occasionally runs longer. For this service, we charge an office fee of $ 16.00.
